Abstract

Since Loeffler described a syndrome characterized by trancient pulmonary infiltration
and eosinophilia with mild or no syptom in 1932, the phenomenon has been considered
by marry investigators as a hypersensivity reaction.
Although paragonimiasis is endemic in Korea, association with Loeffler's syndrome is
rare. This is to report a case of Loeffler's syndrome with unusually high percentage of
eosinophile which was proved to be due to paragonimus.
A 16 year old boy complained of cough, rusty sputum, fever and bilateral flank pain.
Serial chest X-ray revealed evanesrent pulmonary infiltration in changing sites. There
was marked increment of eosinophiles in peripheral blood and bone marrow. Skin test
and complement fixation test for P. westermani were strongly positive. The patient was
completely recovered with administration of Bithionol.
